Been told by a doctor to refrain from physical actvity in the past 3 months.

Feel chest pain when doing physical activity.

Felt chest pain when not doing physical activity in the past month.

Lost your balance due to dizziness or lost consciousness.

Have bone or joint problems (for example, back, knee or hip) that could be made worse by a change in your physical activity.

None of the above.

If you have ticked anything other than 'None of the above', please consult with a medical doctor before commencing any exercise programme. I understand that it is my personal responsibility to ensure that I am medically fit and ready to take part in any physical activity and do so entirely at my own risk.

Physical exercise can be strenuous and subject to the risk of serious injury. You are urged to obtain a physical examination from a doctor before participating in any new exercise activity. This waiver and release of liability includes, without limitation, all injuries which may occur as a result of: (a) your participation in any activity or personal training session and (b) instruction, training, supervision, or dietary recommendations by Motiv8 Fitness

You acknowledge that you have carefully read this “waiver and release” and fully understand that it is a release of liability. You expressly agree to release and discharge your Motiv8 Fitness Trainer from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against your Motiv8 Fitness Trainer for personal injury or property damage.View Liability Release
